Tips for using your Personal Choice 65 PPO plan in 2023

  1. Know your network. As a Personal Choice 65 PPO member, you have the freedom to choose any doctor or hospital in or out of the network. But you'll pay less by selecting a doctor or hospital in the network.
  2. Consider choosing a primary care physician (PCP). Though it isn't required, you may want to select a plan physician as your PCP. Your PCP is the person who will coordinate your care. Regardless of whether you choose a PCP or not, you don't need a referral to visit a specialist.
  3. Dental, hearing, and vision coverage are included. We've got you covered for routine care and more.
    • $0 copay for routine dental, hearing, and vision exams
    • Generous dental allowance (combined in and out of network) every year for fillings, root canals, crowns, partial bridges, dentures, and more
    • Unlimited hearing aid fittings and evaluations with up to two hearing aids every year (one per ear, per year) if provided by a TruHearing provider
    • One pair of eyeglass frames and lenses or one pair of contact lenses are covered every year, up to the covered allowance amount when purchased from Visionworks® or a Davis Vision provider
  4. Save with your IBX Care Card. Lower your out-of-pocket costs by using your IBX Care Card to buy eligible over-the-counter (OTC) health and wellness items at participating retail locations, including CVS, Walgreens, Walmart, and more. Simply swipe your card at the register — it's that easy! Your IBX Care Card is prepaid and automatically reloads each quarter. Please keep your card in a safe location, as you may use the same card for as long as you remain a member of a participating plan.   5. Emergency and urgent care. You're covered for emergency care no matter where you are, but you can save time and money by using telemedicine, a retail clinic, or an urgent care center for non-emergencies. These are great options for minor injuries and illnesses when your doctor isn't available.
  6. MDLIVE® Telemedicine services — $0 copay! You have access to board-certified doctors for medical, behavioral health, and dermatology telemedicine services through MDLIVE. Connect virtually from the comfort of your home via your computer, tablet, or smartphone. Access to the MDLIVE secure portal and scheduling support is available 24/7, 365 days per year.   7. Coverage that travels with you. You are eligible for the optional visitor/traveler benefit, which lets you take extended visits (up to 12 months) to places in the shared Blue Cross®/Blue Shield® visitor/traveler service area. If you visit participating providers in these 48 states and two territories, you'll pay the same cost-sharing amounts as if you received covered benefits from in-network providers at home. Participating territories include Puerto Rico and the District of Columbia. States that are not participating are Alaska and Wyoming.
