How do I become a Patina patient?

If you're new to Medicare, or switching to an Independence Medicare Advantage plan, you can simply select Patina as your primary care physician (PCP) when you enroll.

Follow these steps to enroll online and select Patina as your PCP.

  1. Click here and go through the online enrollment process.
  2. When you reach the Plan Details section of the application, you will be prompted to select a PCP.
  3. Answer Yes to the Do you wish to specify a primary care physician now? question.
  4. Under the search bar pop up, select Manual PCP entry.
  5. Enter 004707223 into the PCP ID field.
  6. Enter Patina Medical Group PC into the PCP Name field.
  7. Answer No to the Are you currently a patient of this provider? question.

If you're already an Independence Medicare Advantage member, you can select or change your PCP to Patina in either of two ways:

  • Log in or register at ibx.com. Click on the My Care tab and select Choose my Primary Care Physician from the drop-down menu. Select Patina Medical Group.
  • Call our Member Help Team at the number on the back of your member ID card.
